*In Part 2 of the Confirmation Packet, the BSA Health Form to be filled out is shown first, then helpful hints and directions in filling it out follows. Please be aware of these instructions. 

Each camper will receive an email with information pertaining to the Scout's registration (balance due, health form parts needed, shirt size, etc...). A link to this page to download the confirmation packets above will be provided. The forms below are also included individually for quick access.
